Over budget and beyond the date of delivery in most projects wasn't a poor planning or poor design instead a culture where workers were rewarded without the skills to perform and popularity was earned by the few who gained favor with the hiring manager.
Focused effort was directed at acquiring a skills matrix aligned with phases and deliverables.
