This quote is from the terrific book "Switch: How To Change Things When Change Is Hard" by Chip Heath and Dan Heath (2010), p. 78 in the hardcover edition. Presented as a case study in change, I thing it also illustrates what can happen in medicine on the macro level when you start thinking of consciousness as a part to be included in the system of care (although I don't know if Dr. Esserman was thinking consciously of consciousness in the way I'm suggesting here).
I pulled the quote from the Google Books version [1].
