Heroin users already come out in the open
Heroin users come out into the open already. Nearly all heroin users I've run into will come to treatment and other services when they feel like it.  Those that don't come out, are usually those who have incurred legal charges in the past. 
They fear getting picked up on warrants. They fear police finding out they're in treatment. This isn't because they fear treatment agencies will turn them in (because, they won't) it's because they fear reconnecting with family and friends or seeing old friends whom they've avoided will cause word to get to the police.
If they're sober and working a program, their life is much more predictable. If they're still using, they are random and harder to catch up with.
