A quest for a real-world example

While the ideas of Thrivability and Systemic Innovation are very rich concepts indeed, they are sometimes difficult to grasp regarding their practical implementation. What real-world example (even a small-scale one) coud be given, along with performance or success criteria (cultural change, rate of adoption, etc.)? What were the involved processes?
