Disturbance and/or distress unrelated to environment
What of those who suffer disturbance of consciousness with no apparent connection to their lives? These are the relative few who, in fact, form the focus of the public's attention on the Other they think of as the "mentally ill" and, by association, they contaminate all thinking about those labeled under the old model. They "stand in" for the entire group.
This group speaks directly to the issue that we have no idea how consciousness is connected or arises from the brain or body. We have no idea what is happening with that person. Some define what is happening to themselves as "suffering distress," which suggests that they are self-reflective at some level and some do not.
Those who suffer from their apparently rootless disturbance and potential distress of consciousness need to helped to have their suffering alleviated but not necessarily their disturbance.
